According to the Electronics Boutique website (type Indiana Jones in the Search box and click on the GO! button), the release dates for the game are:

Strategy Guide = November 17, 1999

PC version = November 23, 1999

Playstation version = January 4, 2000

The LucasArts website, however, doesn't have a release date for either version of the game. :(
