I went to a professional tailor wearing a casual shirt with a collar. I told the tailor why I was getting measured and she measured my chest, neck, waist and arm length (center of shoulders to wrist). She also had me try on several suit jackets to determine my size. I'm glad she did that, because I'm tall and ended up need a long size. I'd recommend having a tailor do it instead of guessing!
