I can only answer this part. I am a 15 & 1/2 inch neck and an off the rack 40". A medium fits great. Sleeves the right length but not too loose or baggy. A fitted shirt in all ways. Shame about the un-finished hem though. Remember if you are UK based (do I remember correctly that you are..?) to add on the dreaded 17.5% VAT as it's not included in the £55 quoted price.
