I've pondered this myself, and for awhile now I've had a few ideas about how to make--not just purchase--one of these books.
First of all, size doesn't matter [insert sick reference here]. Since there is no added flair or bevel to the edges of the notebook, you can easily scale down a larger book of a similar color and binding by just taking a saw to it.
And about the binding: with a little superglue, one could pull of a notebook's cover and binding, pull out some pages to achieve the right width of the notebook, and then rebind it with the glue--with a new cover if necessary.
And on the subject of colors: I've always assumed it would be easier just to find a grey notebook and put a piece of red matte finish tape [slimmed down if neccessary] over the binding. Or, I suppose, you could use paint to get the same effect. Be sure though, that you try to avoid any shininess or glare!
