As far as the shirt, I own a M L one and it fits me fine (I'm 5'11"), except for the sleeves, which are a bit longer than in a regular dress shirt. Actually, it's a great feature, because the shirt doesn't ride up and out of your Indy pants when you put your arms up - works sort of like gussets.
