As Micah pointed out, they use something called a squib. A squib, if memory serves me correctly, is a fake-blood filled condom, with a very very small explosive attached to it. This squib is then attached to the actor, usually below a layer of thin clothing such as a shirt, and remotely detonated when a gun filled with blanks goes off, making appear as if they've been shot. If you want a really good example of this, watch Saving Private Ryan, in the opening beach invasion, the scene where Tom Hanks his holding a young man by the arm, waist deep in water, the young man get shot in the chest(whilst in mid-sentence). Through the wet clothing, you can clearly see the outline of the squib. Hope this unecessaryly long explaination helped.
