: It sounds like we are about the same size. I wear a 16 1/2 shirt, also, but with 34-35 sleeves. I also wear a size 42 jacket and a size 44 custom Wested. I have 2 of Noel's Indy shirts in a size large and they fit perfectly. Go with the large! Regards, 29yr.old Indy
